Summary
I have designed and implemented care pathways for atopic eczema and chronic obstructive pulmonary disease (COPD) for Finnish wellbeing services counties (Pohjois-Savon hyvinvointialue, Keski-Suomen hyvinvointialue and Pohjois-Pohjanmaan hyvinvointialue) together with service designer Ronja Sockolnicki. The goal of these care pathway projects has been to make information on disease treatment and regional care practices easier to find, so that every healthcare professional can access the information relevant to their role. The atopic eczema and COPD pathways also incorporate the patient’s perspective, helping professionals understand what information and support patients need at each stage of their care.

Description
The final care pathways were developed based on interviews with patients and healthcare professionals. The content was written by the professionals themselves, while the designers managed its production and entered it into the technical platform. The care pathways are now used in the participating wellbeing services counties, both in treating these diseases and as a professional operating model. The visual user interface was designed by graphic designer Mika Hautamäki.
